OPTION 1
Complete form online and email

Click on the link to download the file, save the form on your computer, then open it directly in Acrobat or Acrobat Reader.
(Click here to install Acrobat Reader).

Next, click Fill & Sign (right side of screen) and place cursor on the form to enter your text.

To add a signature, click the Sign icon at the top of the screen.

Save the file for your records.

Then click Send a Copy, create link and copy the link.

Open your email and paste the form link into a new email.
